The creative use of biblical stories in the case of David, or the use of paintings and poems in the case of Rachel, portrayed the inner reality of these patients. David's violent and incestuous biblical stories reflected his world of incestuous and destructive wishes towards his primary objects (and towards the therapist in the transference). Rachel's paintings and poems conveyed her unconscious conflicts, depressive fantasies and anxieties, stemming from her fusion with her mother who was a child Holocaust survivor. Working through their relationships with their primary objects and their self perception, as revealed by these creative activities in analysis, facilitated the patients' mourning. The work of mourning incurred in this process helped both David and Rachel achieve a better perception of reality and of their self, unite the fragmented aspects of the self into a whole, and improve their relationships with their primary objects.The book concludes with the therapist's role in facilitating the creative process, which in turn facilitates the therapeutic process. It contains a summary of the role of creative activity in treatment, a discussion of some of the ways in which the therapist  facilitated the creative process in order to arrive at the amazing discoveries which it engenders, and a discussion of the way in which the patients' creativity was successfully incorporated  in their treatment.The therapist's awareness of the patients' creativity and its important role in solving the patients' conflicting life and death wishes enabled her to help them recognize their aggressive and destructive wishes and work them through.  Both patients emerged from a long analysis better integrated.",

However, Marion Oliner counters this potential conflict with her innovative theoretical integration, combined with remarkable conceptual outcomes and treatment techniques.This book spans the author's work over the last fifteen years on the impact of external reality on psychic reality. During this period many analysts, especially in the English-speaking countries and Germany, where historic events loomed large in the lives of their patients, have turned from the exclusive emphasis on psychic reality to greater attention to the traumatic impact of external reality. Considering that this has led to a body of psychoanalytic writings in which events are used to give a name to the pathology, incest survivor, Holocaust survivor, transmission of trauma, to name a few, it has implicitly created two categories of patients: patients who, because of their failed solutions for conflict, are regarded as active agents in their own suffering, and those who are victimized by events they endured passively; thus implicitly taking away from the second group the focus on conflicting motivations. This in turn has led to the adoption of some of Freud's concepts that lack a dynamic dimension. First among those is the repetition compulsion which supposedly causes events to be repeated because they happened. The concept has its place, but, if not properly understood, risks by-passing the analysis of unconscious guilt as a motivating factor in repetition. These factors have not been sufficiently explored in the analytic literature, and over the years the author has written a number of articles that try to distinguish important elements that contribute to the psychoanalytic exploration of trauma. This book is an important summation and further development of that work.",

In addition, high technical content, experiential-dynamic interventions, including defence re-structuring, emotional maieutics, anxiety regulation, dealing with the Super-ego, connecting corners of the Triangle of Others, and Self- and Other- Re-structuring are introduced and discussed. A coding system used throughout the clinical chapters to clarify the nature, and application, of therapist interventions is described. The conceptualization of 'character hologram' is explained in detail, and illustrated with clinical material. Throughout the book, annotated extracts from real therapy sessions are presented to illustrate characteristics of EDTs in clinical practice and, wherever possible, follow-up is presented. The clinical chapters describe the application of experiential dynamic individual and group approaches within the UK National Health Service and Counselling Services, primarily, but also in other Countries having similar public health services (Holland, Israel and Italy). The relevance of experiential dynamic approaches in providing case management and supervision, and in treating the more complex presentations of common mental health problems is discussed. A research chapter provides an overview of EDT-related research to date. It is argued that EDTs represent a promising integration of a number of therapeutic principles, and their place within current mental health policy in the National Health Service in the UK is outlined.",

Its argument is that envy is not simply attributable to constitutional forces, as Melanie Klein proposed, but the outcome of a complex process that includes a disturbance in symbolic functioning. This is the first time a critical review has been undertaken in book from of this cornerstone of British psychoanalysis.As the concept of envy needs to be explored in the light of attachment theory, an important aim of this book is in bridging attachment theory and classic psychoanalytic understanding of severe psychopathology. It also offers, for the first time, not only a reconceptualisation of the notion of envy, but a working model of development which is highly relevant to clinical practice. This model incorporates recent findings from neuroscience, which indicate that environmental influences are of prime importance to infantile development, and that disturbed attachments result in anatomical, physiological and psychological developmental disturbances. The model will be particularly useful in furthering our understanding of the influence on later mental health of an infant's healthy attachment to its mother. From direct clinical experience in the forensic field of psychotherapy, and general practice, the author describes how psychosis and criminal behaviour represent a fundamentally acute failure in the capacity for social interdependence and altruism in human nature that is taken for granted in the psychologically well.Conducting her own research, the author was enabled to examine further the underlying causal mechanisms of aggression and destructiveness in relation to envy. A substantial amount of clinical material in this book supports the author's argument that innate destructiveness is not the primary problem: survival needs are primary, with aggression a secondary reaction when libidinal needs are frustrated. Fight and flight responses in psychopathology, including psychoses, dissociative disorders and perverse activities, are therefore seen as self preservative and not death wishes.The result is a useful paradigm of mental health which does not rely on omnipotent phantasy and which does justice to the importance of human interdependence on the one hand, and adaptability and inventiveness on the other.",

                "description": "There has been a renewed interest in the last ten years in the underpinnings - theoretical, philosophical, and historical - of the Gestalt approach. Often in the past, these have been lost in oversimplified versions of the therapy. The author's aim in his writings has been to provide a full and coherent account of Gestalt theory, and to emphasise our links to our therapeutic and philosophical heritage, particularly psychoanalysis and existentialism. His  theme is a field-relational theory of self as the centrepiece of the approach, and how this has been placed within a structure that is still recognisably psychoanalytic. In this approach, self is understood as meaningful only in relation to what is taken as other, and how that other is contacted. The formation of a relatively coherent self-concept is a task, not a given, and can be problematic as well as helpful (when it no longer supports the person's life-situation). Thus therapy is not an attunement to a self inherent in the client, but an exploration of contacting and awareness; and the therapist's stance can never truly be seen as neutral. Many of these ideas have found their way in some form into other therapeutic approaches (Intersubjectivity Theory, Dialectical Behaviour Therapy), and the actual relationship between therapist and client is acknowledged as highly significant. However, this has usually happened without the underpinning of a systematic field-relational approach to psychotherapy, and Gestalt Therapy, which has one, has for historical reasons not been in a position to engage with these developments.
